Dedication Request Forms
Dedication of altar flowers or the sanctuary lamp is a thoughtful way to honor a special people or events in your life. To place your reservation, please complete the applicable online form below. (Scroll down the page to access a printable form to download instead.) Altar flowers cost $35 for one Sunday and the Sanctuary Lamp costs $7 for the week. Please contact the church office with any questions.
Note that it is not necessary to give flowers or the lamp in order to have people's names added to the Sunday bulletin for prayers or thanksgiving. To add a name (or names) to the bulletin, please contact the church office. In addition, prayers may be added during the service (aloud or silently) during the Prayers of the People.
